Dr. Lindsey's Newsletter #45: Your Guide to Thigh Surgery After Ozempic

Welcome back to our series on body contouring after significant weight loss! For those of you that are joining as a new reader I want to quickly cover the principles of body changes after weight loss.
Shedding excess weight has many health benefits, but unfortunately the damage that is done to skin during weight gain is permanent. The skin envelope of your body grows with fat expansion. After fat loss the skin envelope will retract within limitations. If you frequently read my newsletters you will know my analogy of stretched out and broken Spanx. Healthy, young skin has a Spanx-like quality from a healthy matrix of collagen and elastin. Much like a Spanx garment, this strong elastic structure conforms to the body tightly and results in a smooth contour.
The changes that occur over time and with significant weight changes are explained by three underlying principles:
- With weight gain the Spanx of the skin is expanded, stretched, and in the case of stretch marks - broken.
- With age collagen and elastin decreases - these are the strength and stretch building blocks of your skin Spanx.
- Genetics matter.
This means that the younger you are, the smaller weight changes you have, and the more favorable your genetic code, the better your skin quality. Although we cannot change the written genetic code of our bodies, we can certainly influence and effect the way our DNA is expressed with diet and exercise. If this concept is new to you I encourage you to familiarize yourself with the field of epigenetics. Epigenetic changes can be influenced by various factors such as diet, stress, and exposure to toxins.

Understanding Thighplasty
Thighplasty is an aesthetic surgical procedure that removes excess skin and fat from the thighs. The scar is carefully designed to run along the inner thigh between the groin and the knee. How long this scar is will be determined by how much excess skin is present. The goal of the surgery is to create a smoother, firmer contour that blends with the body above and below. As with all body contouring procedures it is absolutely important that you understand you are committing to a scar in exchange for an improved contour. Most patients are willing to make this commitment because the tradeoff is worth it to them. The thigh lift scar is located in an area that is visible in shorts or swim wear, yet discrete because of the inner thigh placement. Most patients find the improved appearance of the leg to outweigh the presence of a scar. The way clothing fits will improve, and you can even enjoy squeezing into yoga pants without the ripples.
Ideal Candidates for Thighplasty
Thighplasty is an excellent option for individuals who:
- Have achieved significant weight loss and maintained a stable weight.
- Are in good overall health without any medical conditions that impair healing.
- Have realistic expectations about the outcomes of the surgery and are willing to trade poor contour for a scar with better contour.
- Are non-smokers or willing to quit smoking to ensure proper healing.
The Procedure
Prior to surgery I mark my patients with them awake in a private pre-operative room. During this marking session I carefully plan the skin and fat to be excised. My patient is able to see the markings and it helps them understand the surgery and also helps them visualize the scar.
After markings my patients take medication administered by an anesthesiologist to make them comfortable and keep them safe as they are prepared for surgery. I perform thighplasty surgery under general anesthesia and it usually takes several hours, depending on the extent of the surgery, and the unique needs of the patient anatomy. The procedure involves making incisions in the marked area and removing excess skin and fat. The skin envelope is then carefully shaped and contoured over the thigh and closed with sutures.
Recovery and Aftercare
Recovery from thighplasty requires patience and adherence to post-operative care instructions. Here are some key points to keep in mind:
- Rest and Recovery: Plan for adequate rest during the initial recovery period. Swelling and bruising are common and should subside over a few weeks.
- Compression Garments: Wearing compression garments can help minimize swelling and support the healing tissues.
- Follow-Up Visits: Regular follow-up visits are crucial to monitor your healing progress and address any concerns.
- Activity Restrictions: Avoid strenuous activities and heavy lifting for at least 4-6 weeks post-surgery. Light walking is encouraged to promote blood circulation and prevent blood clots.
- Diet: I require my patients to count their protein intake and to supplement as needed. This is a crucial component of recovery as adequate protein is necessary to heal long incisions.
Results and Benefits
The results of thighplasty are usually visible immediately after surgery, with continued improvement as swelling subsides. As the surgeon my work is carefully executed with precision as I plan and perform the surgery. As the patient, your part is to modify your lifestyle in ways that contribute to your overall health and the ability to heal. Even if I do my job expertly it can be undone quickly through nicotine use, poor protein intake, and lack of commitment to garment wear and restrictions. Surgery is not perfect, the human body is not perfect, and certainly the human tissues fail us in ways that are disappointing. This is why we do the absolute most in our power, together as a team, to achieve a fantastic outcome all while minimizing potential risk factors.
